Each year school physicals are conducted for students in Kindergarten, 6th grade, and 11th grade. Parents/guardians are encouraged to have this examination done by their family physician, (at their own expense), as this provides continuity in the child’s medical care.  If your child will be seen by your family physician and is in kindergarten, 6th, or 11th grade, please print the form, have your physician complete it and mail or fax the completed form to the school. This will become a permanent part of your child’s school record.  If you choose to have the school physician complete the physical in school, there will be a notice sent home with your children with information regarding the non-invasive school physical. Parents/Guardians should read the notice, check the appropriate section, sign the form and return it promptly to the School office.

 

School dental examinations are conducted each school year for students in Kindergarten, 3rd grade, and 7th grade. Parents/guardians are encouraged to have this examination done by their family dentist, (at their own expense), as this provides continuity in the child’s dental care.  If your child will be seen by your family dentist and is in kindergarten, 3rd, or 7th grade, please print the form, have your dentist complete it and mail or fax the completed form to the school.  This will become a permanent part of your child’s school record.  If you choose to have the school dentist complete the exam in school, there will be a notice sent home with your children with information regarding the exam. Parents/Guardians should read the notice, check the appropriate section, sign the form and return it promptly to the School office.
